I got locked in the Stygian Spire room in the Ulaman-fight.

I encountered an Ulaman inhabited depth in The Apex of Sacrifice and when Ulaman phased and I killed the second Spire, the door back to the boss-room wouldn't open. The first Spire-door opened just fine.
I had to relog and go back down, to have it happen again in the third Spire-room.
