An unexpected disconnection occurred

As the title say, I've yet to have a day not to get booted to the login screen while I play. This most often happens when teleportation from/to town, and also after choosing a character and entering the game.

Memory: 8192MB RAM
Card name: NVIDIA GeForce GTX 770
Display Memory: 4095 MB
Dedicated Memory: 1989 MB
Shared Memory: 2106 MB
Current Mode: 1920 x 1200 (32 bit) (59Hz)

Running the game at min settings in DX11 [seems more stable/less prone to DC me] at 1400x1050.

I have this error on pretty much every loading screen, game is just unplayeble after 3.0.
