Subject: [PoE2 Bug Report] Lost “Ultimatum” item after defeating Shyklusian, the Chimera
Hello GGG Support Team,
I am reporting a potential bug in Path of Exile 2 regarding a lost quest/unique item called “Ultimatum.”
1) Issue Summary
Location: Chimerid Marshes (a zone I encountered during the “Chaos Trials” questline).
Boss: Shyklusian, the Chimera.
Item Lost: Ultimatum (required for progressing the “Chaos Trials,” based on in-game text).
After defeating Shyklusian, the Chimera, the item Ultimatum dropped. However, my inventory was full at that moment. I attempted to clear space to pick up the item, but it suddenly disappeared from the ground and did not appear in my inventory, stash, or any other storage.
2) Steps to Reproduce
Enter the Chimerid Marshes during the Chaos Trials quest.
Defeat Shyklusian, the Chimera.
Observe the “Ultimatum” item drop onto the ground.
Have a full inventory, so the item cannot be immediately picked up.
Attempt to make space in inventory or reopen the zone.
Item disappears unexpectedly and cannot be found anywhere (on the ground, in stash, or quest inventory).
3) Observed Result
The Ultimatum item vanished completely once I tried to free up space.
Reloading the instance, checking stash, or creating a new instance did not restore the drop.
Because this item is (apparently) required to continue or complete the Chaos Trials, I am now stuck and cannot progress further in this quest line.
4) Expected Result
I would expect that a quest- or boss-specific drop remains on the ground until I pick it up, or is retrievable if my inventory was full.
If the item is essential for progression, there should be a way to obtain it again (either by defeating the boss a second time or via a backup mechanic/NPC).
5) Additional Attempts & Notes
I tried re-entering the Chimerid Marshes (via Ctrl+Click on the waypoint/entrance) to see if the boss or item would respawn, but neither reappeared.
I searched every stash tab, my inventory, and any other possible in-game mailbox or “lost items” feature (if any exists). The item is not there.
I have no way to reset or repeat the Chaos Trials quest to get Ultimatum again.
6) Request for Assistance
Could you please investigate this bug and/or restore the Ultimatum item to my character? If there is a workaround or a way to reacquire it in-game, I would greatly appreciate any guidance. The inability to proceed with the Chaos Trials quest is blocking my progress in PoE2.

I encountered the same problem and hope it can be solved

It's sad, because of this, I can't complete the side mission and fully enjoy the game! 😢
