Omen of Amelioration not working when paired with other omens

Steps:

1. Equip the following Omens and activate them in you inventory:

- omen of amelioration
- omen of resurgence
- omen of refreshment

Got an itch on my nose, and by the time I relieved it, a few small monsters killed me.

Was horrified to see my 10% experience gone. Looking in the inventory, I could still see the orbs of amelioration/resurgence still active, with the refreshment one gone.

Times reproduced: 1 - not like I will try this again, so I removed the other 2 omens and will play like that.

This is intended design. Note that those three omens state that "only one omen can be triggered from combat in each instance.". In other words, you reached low life and had the omen of refreshement activate, which subsequently prevented the omen of amelioration from activating upon death, or the omen of resurgence from activating simultaneously. It's a mechanic designed to prevent abusing large numbers of omens of resurgence to repeatedly heal yourself upon reaching low life.

I would recommend only keeping an omen of amelioration active unless you feel confident enough to rely on the omen of resurgence to avoid a death in the first place- the omen of refreshment is not very useful, in my opinion. Sorry about your experience- the text explaining this is easy to miss.
