Queen of Filth didn't drop quest item "Temple Door Icon"

Good day,
I just beat the Queen of Filth in act 3 cruel, but she didn't drop anything. No random loot and not the required temple door idol.

I did not die during this this try and not since clearing her. After waiting for a few minutes I tried changing maps and reloading from checkpoint but neither helped.

She just lies there, in her pond, dead and not giving me my shit I worked so hard for.

Please fix.
