4-th ascension drop bug??

Hello

Completed inscribed ultimatum 75+ 3 times to get the fragments to enter the trial master boss fight.

Killed the trialmaster and for some stupid reason i thought you get the ascension points automatically (later found out that you needed to interact with the altar nearby from a youtube video....)

Now i have done the 75+ 10 trial runs 5 times and every time iv gotten the same fragment.

So technically i cant fight the trial master again....

Is it a bug or just very bad luck?
