How did I die? 89% ES left upon death

I need help understanding how I died in THE DROWNED CITY Hardcore Act 3 Cruel.

Had on Chaos Inoculation and no items equipped that took life away from me.
Halfway through the map I get slowed down for a second and instantly die.

Is there a way to revive my character being that I still had ES available. Is there some random glitch that occurs that bypasses the ES and attacks life when Chaos Inoculation is on?

Devs please help me out here. I love this game and have over 270 hours invested. This is unacceptable in my opinion.

Anyone has some helpful insight I would greatly appreciate it.
