[BUG] - Instance Crash/Disconnect/Kicked for "too many actions"
This has happened to me multiple times now. I'm playing an ED/contagion lich. Contagion + totem can create a situation with lots of mobs on the screen where the game simply locks up/kicks you. When you return, all portals will be gone, map will be marked 'failed', and you will lose all loot in your inventory you collected from the map.

The same thing happened to me today. All portals are gone, and I lost my loot, including a unique tablet I had. Please fix this

This is happening over and over, to the point where I can't even play the game. Are we aware of the cause of this issue? I believe it has to do with Lich and effigies.

" Not a bug look at action limit I guess its pretty much the same for poe2 https://www.poewiki.net/wiki/Action_speed Dot's spreading to an insane amount of monsters while the totems target all of them as well as Blasphemi applying the Curse to everything can reach this limit relatively fast. It's a protection measure to prevent the servers to no not go from ticks per seconds to seconds to tick. I have a char with the same build, just spread the totems out more in very high density mob packs and you will be fine. I only get kicked when I overdo it like opening a high density expedition with the 1 explosive passive and applying Contagion and Essence drain as well as placing 2 totems at close distance to everything. If Contagion splits to a lot of mobs in a very short of time the time to calculate damage gets out of hand very quickly. What you're saying makes no logical sense. I'm playing the game as intended. If doing so causes the came to crash, that's a bug.
