Simulacrum bug - not opening wave 16

I was playing until round 15. After I've beat the round, suddenly the fog dissipated and it seem like simulacrum has ended. The Entity was not there so I left the simulacrum via portal and came back in. Now the Entity is back, but the fog is still dissipated. When I click to start new wave, nothing happens but entity glows when I mouse over as though I could click it.

This is the part of the simulacrum that looks like executioners boss space from act 1. When I move to the door separating areas of the simulacrum, fog fades back in when standing by the door. Dissipates again when I move from the door.

Nice waste of simulacrum.

is there a wave 16?

Simulacrum is only 15 waves. The entity being back after you leave and return must be a bug or some part of a future patch adding waves that isn't supposed to be in the game yet.
