I have experienced a lot of "out of map" situations recently like -

Fight a Ritual on a Bridge. When i spawn the enemies from the Ritual and got the restricted area some of the mobs (or just 1) is out of the map (under the bridge not out of restricted area.) and that blocks the whole map from completion.
