Well of Souls - Could not generate Mod [0.3.0c]

I just added desecrated affix to this armour and some other items.
All other items added, revealed affix normally.

Once it showed one affix on the center, but i did it again and it still doesn't show anything and the chat message is "Could not generate Mod"

Getting the same issue in 0.4 now
