Dying on map boss resets map

Was in a normal t15 map. Not corrupt, not nexus, not unique. Had boss, expedition, breach. Map 90% done, still had abyssal depths, expedition, 3 rares. Got to the boss at end. Died, first death. Instantly respawned, map reset, all fog of war, no mobs, no modifiers. All bonusses stripped.
