Hammer of Kamasa not dropping – quest blocked (Bug ref: 500844869)

Hello,

I encountered a bug with the quest item Hammer of Kamasa.

Character: oneshootq

League: rise of the abyssal leauge

Patch version: 0.3.0

After defeating Mektul, the Forgemaster in the Molten Vault, the quest log still says "Pick up the Hammer of Kamasa", but the item never dropped.
I tried:

Resetting the instance multiple times (Ctrl+click → New).

Relogging.

Checking inventory and stash.

The quest is now blocked, since I cannot proceed without this item.

Important detail:
I already have another character in the same league/season that successfully obtained the Hammer of Kamasa. On this second character, the quest item does not drop at all. This might be connected to the account-wide unlock of the crafting bench from this quest.

Additional note:
I suspect the main reason the Hammer didn’t drop this time was that I died during the fight, and the boss probably died while I was dead. That might be why the item didn’t drop.
