Breaches ends up outside map boundaries

Breaches ends up outside the map where we can't go.
This has happened so many times now. Also has happened with myself when using a headhunter that there was a mob in one of these spots and headhunter teleported me to the mob and I got stuck down there without a way back losing the whole map.

Please either remove these unreachable spots or make them reachable.
