Bug at the end of act 4 end boss last phase when killed

Hi,

I did many attempts killing the last boss act 4 (tavakai). Once i killed him at the edge of the arena the hooded one spawned in and then it bugged out by not being able to interact with him.https://imgur.com/a/M0jt9e3

teleporting to base and back did display an animation of the hooded one with the sword doing something but didn't progress the quest state at all.

the way i fixed it for myself is entering a new instance and do the part over again, but i think this should be fixed because its a 3 phase boss fight with no in between spawn points and its pretty hard at the end.

I got stuck similarly. Makoru was apprearing on the ship at kingsmarch without being revived.

I did the First herald again got the weapon again then went and killed tavakai again
