Constant crashing when shapeshifting

I encounter pretty reliable crashing on PS5 when I go from bear og wyvern form to human form.

This bug started at the Jinquani's machinarium where I needed the soul cores.
If I transform to animal and then do anything (including interacting the the soul core receptacle) I crash.

Now it happens everywhere and I am unable to progress the game as I have no more skill gems etc.

I also sometimes crash just staying in animal form without doing anything out of the ordinary skill wise.
