Bugged Progress Quest - Mysterious Shade and Lachlann Ring Absent

Defeated both Cemetary Eternals Boss, proceeded to open gate to fight Lachlann Boss and Loot Reward Ring. Lachlann NPC Boss absent, gate is opened with green item keys consumed. No Lachlann, and no ring drop and no visible quest to track.

Proceeded to next map area Hunting Grounds.

Mysterious Shade aka The Hooded One is not revived and cannot be interacted normally.
