Recruit the Vaal

The twin bosses during the quest recruit the vaal (act 4) gtes bugged if you failed the fight. In order the retry the fight, you will need to go to a new instance of the map, meaning clearing the map again (and again if you fail again).

I concur. Happened to me as well.
