Shader Crash [XSX]

XSX, couch co-op.

Changed a skill gem out and it appears the shaders all crashed. Can't move, can't interact, but can open the menu and quit the game manually.
Happens several times per hour, and I can't quite figure out what the trigger is.

This time it was a skill gem swap and closing the menu. A few minutes ago, it was trying a different skill.
