Vaal Beacon miniboss + Vaal Temple medaillons

I’ve noticed several times that a Vaal miniboss at the Vaal Beacons (maps) seems to be bugged. I don’t know the name of the miniboss, but it’s a fire-based one that shoots fire. The bug occurs when it burrows underground. Small eruptions appear in the ground, and then nothing happens anymore.

Additionally, in the Vaal Temple, for most medallions with “normal rooms,” their effects are not shown when I try to place them in the temple. The spots where I can place the medallion rooms are highlighted, but the room itself and its effect on neighboring rooms (tier/level) are not displayed.
