avakai stops following/healing in Act 4-5 Maps

Hello. I have a persistent issue with Tavakai in Act 4 and 5. He frequently stops moving and stops healing my character (Sorceress).
Used Town Portal (TP) to reset him — did not work.

Relogged into the game — did not work.

The only temporary fix is creating a "New Instance" via Ctrl+Click, but the bug returns.
