It is real to my knowledge. It was a bug from last year, if I remember correctly.

Assuming this is real, the only useful observation I can make from the screenshot is that it appears the player has only killed 1(?) mob based on his vaal skill charges.
So perhaps if you find and kill ONLY an albino rhoa in a mud flats instance, you get a uniquesplosion? Or maybe a 1/100 chance per albino rhoa. Step 1) Run atziri until you get bored and stash tabs of uniques
Step 2) kill a mob then run back and forth to town dropping all your uniques on the ground around the mob Step 3) post pics Step 4) commence troll posts IGN: OldManBalls (Warbands)

There's also the unique Slink boots (Atziri's Step) in the middle of the left side. So that screen can't be from last year.
