Capture Master of the Blade

I decided to complete the Soul of Lunaris Pantheon. I purchased 3 divine vessels, scrounged thru my stash tabs for Sewer, Ashen Wood and Lair. Ensured that for each map I put in an empty vessel, and took out a full vessel for each of the aforementioned maps. Went to Sin in Act 9, and was only reward with Arachnoxia and Lycius.
Master of the Blade was NOT rewarded to me. Unfortunately, I have no proof to support my claim. This just happened to me, too. This was my first attempt ever at capturing a soul, so I spent a long time trying to figure out what I could possibly have done wrong, since I wasn't sure how the procedure would go.
