You're using Omen on the Winds jewel which breaks chain, take it out you'll start hitting a lot more things, giving you way more life on hit. Also always have Vaal Grace running with metamorphs to max your dodge I make odd builds and sometimes write guides. I think it might be your ice bite linked to herald making it reserve more than 25%. I had issues when I accidentally linked something to an aura making me have too little mana (like less than 100) and a 6 link ice shot is like 67 mana per shot.
