Cannot progress past Ogham village

I kill the Executioner, save the girl (Leitis i guess) than dont go through portal to the Manor rampants, but port back to city with loot. Here i got disconnect.
Now when i port back to Ogham village and kill the Executioner again, there is no girl to be saved cause she is already in my city and portal to next location (Manor rampants) is not open. So i cannot progress any further.
