Rake sometimes misses and/or doesn't cause bleed

I'm currently in T13 maps. Rake has 100% bleed chance, yet sometimes it will just do some damage with no bleed. Sometimes it does no damage at all so I assume it's missing. My stats say it should have 100% accuracy, yet it seems to be around 75% mark. I hope it's not supposed to be like this because that would make rake an unviable option for late game. Because what happens is I don't interrupt that one white mob by missing and he just punches me in the face for a big chunk of my health if not out right killing me.

Also while we're here. I play on controller. Sometimes rake will just refuse to dash at enemies and I just stand there swinging at nothing. This happens frequently. I tend to use the right stick to pick the direction but at times it seems to want to go where my left stick is going.

Bloodhunt also seems to have the same problem as above.

Just respecced a character to a bleed build, not applying bleed with anything. no rake bleed, no spearfield with lacerate and incision, herald of blood not working.. nothing
