"Demon of Atzoatl" - lore interpretation?

Given that we did incursions, which were trips to the past, where our POE1 exiles slaughtered countless Vaal in Atzoatl, right?

And, now, our POE2 exiles are running through these areas like a bad burrito...

Think of it from Doryani's POV. These two sets of events are within a couple years of each other at most. Might even be less than 6 months for HIM. Sure, for our exiles, completely unrelated, and decades apart. From his perspective, the news coming to his ears is that a Non-Vaal intruder of extreme power is ransacking temples, slaughtering Vaal ruthlessly.

And, now, there is an extremely powerful non-Vaal is blowing through and slaughtering the Vaal HERE.

Then, add to both of these that a woman bearing Alva's description has no doubt been passed around (because certainly before WE started doing incursions, she was experimenting, might have managed to pop back for a few seconds). Now that same woman is running through town WITH that non-Vaal person?

No doubt he would confuse the situation, think same people (Exile + Alva).
