The Silent Cave Incomplete

I completed The Silent Cave on a t15 rare map and I did not get a book drop. The map objective did not complete when I killed the boss in the center, and I was not able to make portals in the boss room after killing him. I also got an incomplete red dot on the map after finally moving on to a new map. It did say 0 monsters remain, and I did kill all rare essence monsters before starting the boss fight.
