Omen of Amelioration not triggered on death!

HI, i have noticed after last patch that the Omen of Amelioration is not working properly, it is not triggered after death, till now, was not triggered in breach ring, delirium fog, some skills from monster in expedition!

pls guy at GGG take a look after it! Thanks

Have you already used any other Omen in the area? Only one can be used per combat zone.

yes was the only 1 active, no other Omen, i see other people have same problem!
