There are still about 80 effects not in the wiki. Also see the list compiled on http://poe.rivsoft.net/shrines/shrines.js
I started combining the information but it got to be too much work.

Some notable effects (from http://poe.rivsoft.net/shrines/list & reddit):

You catch a faint smell of corruption on the wind
Corrupting tempest
Item with spell damage (& attack speed)? There were only 2 linked items in the data.

An accurate map makes for a rewarding journey
You are rewarded with 20 Cartographer's Chisels
Jewel with shock

Keeper of wealth
one mob drops a large amount of currency
Jewel with totem damage (or totem resist?)

Leave no stone unturned
check all clickables in the area
Jewel with max life

Become the nightmare
divine shrine effect
Map with elemental reflect? (only 1 item was submitted so this is a just a guess)

Seek the Apex
sacrifice/mortal fragment(s)
Jewel with Mine (laying speed or damage)


Please press Ctrl-C while hovering over your item before sacrificing. If you get in interesting effect submit it at http://poe.rivsoft.net/shrines/. E.g. getting a cartographer's box (the person on reddit wrote that it was a 1 hand weapon).
