Had the same issue today. I assume you play in windowed or windowed fullscreen mode? Change it to fullscreen. (Options > graphics > display settings > mode > fullscreen) For some weird reason the game can't handle windowed modes very well. I recently get a lot of "Failed to connect to instance". Playing on the Frankfurt (EU) Gateway. Seems to happen on every fourth to fifth zone transition. Been especially bad these last few hours. Sarn Encampment -> The Slums - Failed to connect. Login again, ctrl-click to get a new Instance of The Slums The Slums -> The Crematorium - Failed to connect. This is getting extremely frustrating.
